Terms of Use

Welcome to Elevations. Elevations is a solution for non-medical drivers of health that brings organizations together to provide community, health, educational, and supportive services to individuals in the local community. The administrator for Elevations is the Stephen F. Austin Community Health Center, Inc. d/b/a Community Health Network. The organization with which you are affiliated is a participant in Elevations (your “Participating Organization”).

This is our Terms of Use. It describes for you the terms, conditions, and rules applicable to your use of and access to Elevations. This Terms of Use does not apply to any website, service, or application you use to access Elevations. This Terms of Use is a legally binding agreement between Elevations and you, so please read it carefully. If you do not agree to terms of this Terms of Use, you may not access or use Elevations.

1. Use of Elevations

You must be authorized by your Participating Organization to access Elevations and you may use Elevations for only the purposes for which you were granted access by your Participating Organization. You must comply with this Terms of Use when accessing and using Elevations.

2. Access to Data

You may access and view only the information of those individuals with whom your Participating Organization has, had, or reasonably anticipates having a professional relationship (“Client Information”). You must: (i)Request, access, and view only the minimum Client Information reasonably necessary to permit you to perform your professional role and tasks in a timely, efficient, and effective manner; (ii)Not request, access, or view personal information that is not Client Information; (iii)Comply with Elevations current access policies and procedures.

3. Safeguards

You must take reasonable precautions and employ reasonable safeguards when accessing Elevations, including, but not limited to:(i)Maintaining the confidentiality of computer terminal displays by never leaving the terminal unattended or available for unauthorized viewing when Elevations is being accessed; (ii)Limiting access to locations that permit secure viewing; (iii)Not sharing Elevations account user names, passwords, or other log-in credentials.

4. Acceptable Use

Elevations has established certain basic requirements for the use of Elevations and we require you to adhere to them. The purpose of these requirements is to set forth how you may use Elevations and certain behavior and actions that are prohibited. When using Elevations, you are not allowed to upload or share anything that:

• Is sexually explicit or pornographic, creates a genuine risk of physical injury or property damage, or promotes self-harm or drug abuse;
• Attacks, bullies or harasses or includes hate speech;
• Infringes anyone’s intellectual property, privacy or other rights;
• Is fraudulent or deceptive;
• Contains any information or content that is illegal or knowingly false.

Further, you are not allowed to:

• Access, tamper with or use non-public areas of Elevations;
• Break or circumvent our security measures or otherwise test the vulnerability of our systems or networks;
• Use any undocumented or unsupported method to access, search, scrape, download or change Elevations;
• Try to interfere with any other Network user;
• Do anything that violates applicable law or regulations;
• Encourage or help anyone do any of the things on this list.

5. Reporting Inappropriate Use of Elevations

Any inappropriate access to or use of Elevations must be immediately reported to your Participating Organization at support@chntechsolutions.com. Inappropriate use of Elevations may include:

• Sharing your access credentials with a third party;
• Using Elevations for private or personal reasons;
• Accessing Data inappropriately, such as individuals with no relation to your Participating Organization;
• Sharing or uploading inaccurate, unacceptable, or inappropriate documentation or information.

6. Privacy

Use of Elevations is subject to the Privacy Notice that is incorporated into this Terms of Use.

7. Modifications

We may revise this Terms of Use and the Privacy Policy from time to time. If we decide to change our Terms of Use or Privacy Policy, we will post the revised policy on Elevations. If changes are significant and materially affect your rights or use of Elevations, we may provide a more prominent notice on Elevations. In certain cases, we may also provide email notification of the modifications. However, because we may make changes at any time without notifying you, we suggest that you periodically consult this Terms of Use and our Privacy Policy.
